  • Enjoy better health. Schedule annual free preventive care visits and monitor your health to feel better and stay healthy.
  • Spend less. Address health issues before they become serious and save money on health care expenses.
  • Receive a cash benefit. Take simple steps to improve your health and receive extra money in your paycheck.
Three steps

Start Earning $$

  • Get an annual physical exam. After receiving your results, discuss a plan of action with your doctor.
  • Submit your results. Send the completed Physical Exam Verification Form to the Wellness Council of Arizona.
  • Receive your wellness incentive. When your completed form is received and processed, you are eligible for a wellness incentive based on your medical plan.
