Energy for Life Wellness Incentive Program

The UNS wellness incentive program encourages you to:

1. Enjoy better health. Schedule annual no-cost preventive care visits and monitor your health to feel better and stay healthier.
2. Spend less money. Address health issues before they become serious and save money on health care expenses.
3. Receive a cash benefit. Take simple steps to improve your health and receive EXTRA MONEY in your paycheck.

Earn Your Wellness Incentive

1. Get an annual physical exam, including labs, and set wellness goals. Discuss a plan of action and set your wellness goals with your doctor.
2. Submit your results. Send the completed Physical Exam Verification form to the Wellness Council of Arizona.
3. Receive your wellness incentive. When your completed form is received and processed, you are eligible for a wellness incentive based on your medical plan:
   • TEP UEE/UEG Unclassified, UEG 1116, and Unions 387/769: PPO Plan A/B & HDHP members receive $500 ($19.23 per paycheck for the next 12 months)
   • TEP 1116: PPO Plan A/B receive $500 ($19.23 per paycheck for the next 12 months). HDHP members receive $200 ($7.69 per paycheck for the next 12 months)*

*If you change medical plans during Open Enrollment or an eligible event, your cash benefit amount per paycheck may be adjusted accordingly, depending on your company and classification, to the new cash benefit.
