2026 ID CARDS: BCBS Group IDs are changing for 2026 and you will receive new paper cards in the mail (2025 ID cards will no longer be valid). If you prefer a plastic card, please request one through the member portal or Customer Care at 800-752-0193. If you use the BCBS app, digital cards will automatically update to the new Group ID 101570.

With the BCBSAZ PPO medical plans, you receive higher coverage in exchange for higher premiums. For office visits or prescriptions, you pay a copay for in-network services. Both PPO plans cover the same benefits at different levels.
With the BCBSAZ HDHP, you are responsible for paying the full cost of most services, until you satisfy the annual deductible each year. After you meet the deductible, you will begin paying a percentage of the cost for service, called coinsurance. In this case, you are generally responsible for 20% of the cost for care until you reach the plan’s out-of-pocket maximum. Preventive care is always covered at 100%.
All medical plans offer access to the BCBSAZ Statewide/Nationwide PPO network of providers and facilities. While you can visit any doctor, you’ll save the most money by using in-network physicians or facilities.
